If you will be beginning treatment at Athens Therapeutics in the near future, you will need to complete the necessary paperwork prior to your first visit.  These forms are available below for your convenience. Please print out all of the appropriate forms for either adult patients or children, fill them in to the best of your ability and bring them to your first appointment.  If at any time you have questions or concerns, please do not hesitate to contact us.

If you are unable to keep your appointment, please notify us at least 48 hours prior to your scheduled visit.  If you would like for us to verify your insurance benefits prior to your first visit, please mail or fax a copy of the front and back of your insurance card.
